Steady, compassionate support for complex health journeys
RN-led care coordination and navigation for individuals managing chronic illness, multiple providers, or overwhelming medical needs.
What I do
When healthcare feels overwhelming, you don’t have to manage it alone.
I help patients and families:
Make sense of what your doctors are telling you
Keep all your providers on the same page
Stay on track with medications and appointments
Feel more in control and less alone in your care
Who I sERVE
This service may be right for you if:
You’re dealing with ongoing health issues and feel overwhelmed trying to manage everything
You have multiple doctors and it’s hard to keep everyone on the same page
You’re helping care for a loved one and need guidance and support
You’ve recently been in the hospital and aren’t sure what to do next
Led by a Registered Nurse. Grounded in clinical experience, compassion, and patient advocacy.
Hi! I’m Jennifer.
I started Solid Earth Nursing Pathways because I’ve seen how overwhelming healthcare can be, especially when you’re managing multiple conditions, doctors, and medications on your own.
As a nurse, I’ve worked closely with patients who were doing their best to keep up with complex care, often without clear guidance or support. It’s a lot to carry, and too often, people are left to figure it out alone.
This work is about changing that.
I help people feel more steady, informed, and supported as they navigate their health whether that means making sense of a doctor’s visit, staying organized, or simply having someone in their corner.
I have four years of experience in hospital care and two years in case management, working primarily with patients who have higher medical needs and complex care situations.
I earned my Bachelor of Science in Nursing and am currently pursuing a Master of Public Health, with a goal of helping improve how patients and families experience and access care on a larger scale.
Outside of work, I’m a mom of two boys as well as two dogs (Tucker and Poppy) and two cats (Ridley and Hamilton). I live in Franklin and love the slower pace and small-town feel.
