Hi! I’m Jennifer.

I started Solid Earth Nursing Pathways because I’ve seen how overwhelming healthcare can be, especially when you’re managing multiple conditions, doctors, and medications on your own.

As a nurse, I’ve worked closely with patients who were doing their best to keep up with complex care, often without clear guidance or support. It’s a lot to carry, and too often, people are left to figure it out alone.

This work is about changing that.

I help people feel more steady, informed, and supported as they navigate their health whether that means making sense of a doctor’s visit, staying organized, or simply having someone in their corner.

I have four years of experience in hospital care and two years in case management, working primarily with patients who have higher medical needs and complex care situations.

I earned my Bachelor of Science in Nursing and am currently pursuing a Master of Public Health, with a goal of helping improve how patients and families experience and access care on a larger scale.

Outside of work, I’m a mom of two boys as well as two dogs (Tucker and Poppy) and two cats (Ridley and Hamilton). I live in Franklin and love the slower pace and small-town feel.

I offer personalized support to help you navigate your health with more clarity and confidence. Whether you’re managing a chronic condition, recovering after a hospital stay, or feeling overwhelmed by multiple providers and medications, these services are designed to meet you where you are.

Prices are based on out of pocket payment. I am unable to bill insurance at this time.

Prices negotiable based on income or financial need. HSA/FSA eligible.

    • 2 monthly visits including doctor office coordination, education, health assessment, and medication reviews

    • One phone call per week with RN

    • Unlimited messaging on Carepatron

    • Appointment coaching, debriefing

    • Education on health conditions

    • $60/month

    • One check-in per week either call or in person including doctor office coordination, education, health assessment, and medication reviews

    • Unlimited messaging between patient and nurse

    • Coaching and education for complex health conditions

    • Medication management including pill planners and refill management

    • Appointment preparation and debriefing

    • $100/month

    • Two check-ins per week, either call or in person including doctor office coordination, education, health assessment, and medication reviews

    • 2 RN escorted appointmnets per month

    • Unlimited messaging between patient and nurse

    • Coaching and education for complex health conditions

    • Dietary review and meal planning

    • Appointment preparation/debrief

    • Medication management including pill planners and refill management

    • 25% off extra/optional services

    • $150/month

  • Either one time service, add on service, or a build your own monthly service plan.

    • Discharge planning or transition: $75

    • Medication set up (bimonthly): $50/ month

    • Medication management: $75/month

    • Health assessment + coaching: $100

    • Advanced directive planning and notarization: $75

    • Add on visit: $50/hr
